For Link Users

To refer a patient, login to your Link account on the right. Select the patient you are referring and order a referral for the desired specialty or service. Your referral order goes directly into a work queue in our EMR for our referral specialists.

By entering your referral through Link you can track the status of the referral at any time. The referring provider will receive an in-basket notification in Link when the referral is complete.

No Link Account?

If you would like access to your patient's EMR, please follow these three steps: 

  1. COMPLETE THE ONLINE FORM

    First, please complete the online form. Required fields are marked with a red asterisk. This information is necessary so that University of Utah Health can assign your unique user ID.

  2. READ THE ACCESS AGREEMENT

    After you have completed the online form, please read the "Electronic Health Record Access Agreement" at the bottom of the form. If you agree with the terms of the agreement, please check the box next to "I have read and AGREE to these terms and conditions." Then enter your name in the field under the check box and click "Submit." This is a legally binding agreement and covers your responsibilities as a provider who will be accessing protected health information (PHI).

  3. GIVE US THREE TO FIVE DAYS TO SETUP THE ACCOUNT

    Through a secure connection, we will receive your information and agreement to allow us to set up your account. This process will take three to five business days. Once your account is set up, you will receive an e-mail with your login and instructions on how to obtain your password. You will then be ready to access your patients' records.
